Release Forms

Trip Forms: Three-step Process

Download Event Release and Medical Release forms for group events below. You need to fill out both of the forms (PDF files available) for any FCCE camp, trip or outside activity that you want to attend.

STEP #1: Download and print the Event Release form:

STEP #3: Fill out all appropriate forms and return them to the Youth Director along with payment (if needed) for the camp or event.
